A decision has been proposed for the benefit of all patients of Plumstead Health Centre, to merge with Trinity Medical Centre.
This proposed merger will mean that patients at all practices will receive continuity of care and improved access to both core, additional and enhanced services.
The proposed merger would also give long term sustainability and resilience to both practices and also provides continuity of service to the patients of Trinity Medical Centre.
Patient Participation Group Consultation has taken place across both practices, to provide patients with the necessary information on the proposals and an opportunity to raise any concerns and questions.
The merger will provide the required staffing levels, provide adequate premises and help support sustainability, viability and stability for all patients.
